Our professional network is essential for providing continuity of care and ensuring that our patients have the support they need post-treatment. Our clinical team, outreach specialists, and case managers work together onsite and collaborate with our community partners to craft individualized discharge plans for those preparing to leave our care. If you are a medical professional, behavioral health provider, registered dietitian or eating disorder recovery coach and are currently taking new clients, please fill out the following form to be added to our referral list.
